Endoscopic evaluation of the female genital tract can be done abdominally by laparoscopy or vaginally by hysteroscopy. The complete laparoscopic & hysteroscopic assessment is considered the gold standard. Both methods can guarantee the diagnosis of the morphological & functional integrity of the genital tract.

Operative laparoscopy & hysteroscopy can be used to treat pathologies of female genital tract (for fertility enhancing surgeries) and also to remove fibroids & uterus (hysterectomy) in case they are pathological or diseased.

Endoscopy is quick and relatively safe procedure which eliminates the need of conventional open surgery. The risks associated with endoscopy are minimal & the recovery is faster.

Based on the area in which endoscopy is performed it is named as Hysteroscopy & Laparoscopy.

Hysteroscopy

1. Diagnostic Hysteroscopy

Diagnostic hysteroscopy is an incision-free procedure used to evaluate the endometrial cavity. The hysteroscope is 4 to 5 mm in diameter. It is inserted via the neck of the uterus. Hysteroscopy is usually carried out without anesthetic & lasts less than 10 minutes.

2. Operative Hysteroscopy

Operative hysteroscopy may be used to both diagnose and treat certain conditions such as uterine adhesions, septums or fibroids.

It is usually carried out under local, general or regional anesthetic.

Laparoscopy

1. Diagnostic Laparoscopy

Diagnostic laparoscopy is used to evaluate female genital tract. In laparoscopy, A fiber optic telescope, called a laparoscope, is inserted into the female’s abdomen to look for the patency of fallopian tubes, endometriosis, scarring, adhesion, and other pelvic disease.

If the patient has chronic pelvic pains, infertility, heavy periods caused by fibroids, laparoscopy is suggested.

The recovery is fast and the patient usually leaves the hospital after 2 or 3 hours.

2. Operative Laparoscopy

Once disease is identified using diagnostic laparoscopy, operative laparoscopy is performed to treat the disease.

Instruments such as a laser can be useful in some cases to treat adhesions and endometriosis.

It includes procedures like hysterectomy (removal of diseased or pathological uterus), myomectomy (excision of uterine fibroids), adhesiolysis (removal of internal adhesions), ovarian cystectomies, excision of endometriosis.
