Common causes of male infertility

Among all causes of male infertility, the most common is quantity and/or quality of sperms. Common causes of male infertility include:

  • Abnormal sperms due to inflammation of the testicles, abnormally developed testicles or swollen veins in the scrotum.

  • Low sperm count due to infection, injury, hormone disorder, or use of alcohol, tobacco or other drugs.

  • Other causes may include disorders in genitalia, gonads or genetic conditions.

Unfortunately much of male infertility is idiopathic, reflecting our still very poor understanding of the mechanisms that govern testicular function.

Male Fertility Treatments

Male infertility is most often treated by conventional methods that include:

  • Medications to increase sperm production

  • Hormone restoration

  • Avoiding excessive heat, alcohol, tobacco, exposure to radiation & harmful chemicals

  • Avoiding long hot baths & showers

  • Wearing loose underwear

In cases where the conventional methods don’t show expected results, we use innovative therapeutic options such sperm retrieval & reproductive surgery.

Sperm Retrieval

Sperm retrieval procedures are largely performed in men who produce an ejaculate without sperm or who are unable to produce an ejaculate.

In this treatment, a small number of sperms are obtained directly from the epididymis or testicles in a small surgical procedure. This procedure is followed by female fertility treatments such as IVF and ICSI.

1. Percutaneous Epididymal Sperm Aspiration (PESA)

In this procedure, sperms are collected directly from the epididymis. The collected sperms are immediately examined under a microscope for quality. These sperms can be used in IVF & ICSI immediately or frozen for use at a later time.

2. Testicular Sperm Aspiration (TESA)

In TESA, sperms are extracted directly from the testicles. After examination, the sperms obtained are either frozen for subsequent use or prepared using a process of washing and grading. A single sperm is then injected directly into each egg in ICSI.

Reproductive Surgery

When non-surgical and natural remedies fail to improve male fertility, we suggest surgical options to fulfil the dream of having children.

1. Varicocele Repair

Men who experience pain in their testicles when exercising, standing or sitting for a long period of time—may be suffering from varicoceles. Varicocele treatment is done surgically on an outpatient basis using local or general anesthetic.

2. Ejaculatory Duct Resection

Ejaculatory duct obstruction (EDO) is the obstruction of one or both ejaculatory ducts. Ejaculatory Duct Resection is performed through a telescopic device placed through the urethra under spinal or general anesthesia.

3. Vas Deferens or Epididymal Blockage Removal

Infection or traumatic injury to the genital tract can result in scarring and blockage within the tube that conveys sperm. Men who have a blockage in the ducts can undergo surgery to correct the blockage. The surgery is performed as an outpatient and the patient is generally able to return to work in 3-4 days.
